TERMS OF MEMBEARSHIP:
Upon filling out this form you acknowledge that you understand the rules set forth by BEAR NAKED TANNING and shall receive membership rights and privileges within 24-48 hours. Once your memBEARship becomes active Bear Naked Tanning will email you letting you know! All prices, fees and draft dates are guaranteed for the term of your membership and may not be raised unless a new membership is purchased. I understand this is a continuous automatic withdraw each month until I come into the salon and sign a cancellation form in person. Any changes I would like made to my account including updating my credit card on file, address change, membership upgrade or downgrade, freezing or cancellation must be in person with a form signed. 

Bear Naked will be closed on New Years Eve, Easter, Thanksgiving, Christmas Eve and Christmas Day and will have limited hours on New Years Eve, Memorial Day, Independence Day, Labor day and during our two day tanning conference in October each year. Additionally, Bear Naked may be closed at additional times due to weather conditions, emergencies and/or needed repairs or maintenance to the salon which may cause an interruption of your membership. Bear Naked Tanning reserves the right to cancel your membership at any time for any reason.
